What To Do Before You Begin

Review your benefit choices in your Benefits Guide

Make a decision on your benefit selections:
-    Bundle Package Choice
-    Supplemental Benefits

Gather the following dependent information. Having this information handy before starting will help speed up your enrollment process
-    Social Security Number(s)
-    Date(s) of Birth

Choose enrollment preference - online or via telephone

New for 2017 – Your Life, Accidental death and Dismemberment (AD&D and Disability Enrollment Information is now available through Prudential.

To enroll in Voluntary Life and/or AD&D insurance for yourself, your spouse, and/or your child(ren) and/or to make changes to your current elections, you must visit The Prudential site will allow you to view your eligibility and premiums.

As a first time user, you will need to create an account on the Prudential website by following these steps:

1.) Click “Create an Account”
2.) Enter your Social Security Number (not ID number)
3.) Enter your date of birth: MMDDYYYY
4.) Enter Access Code: 52718
5.) Click “Continue”
6.) To protect your personal information, you must create your own personal User ID and Password. Enter a User ID and Password that you will remember – please note, your User ID and Password are case sensitive. Your User ID must contain letters and numbers and be at least six characters long.
7.) Answer the remaining questions. Click “Submit”. Be sure to review your Life, AD&D, and Disability coverage carefully to determine the options and coverages that best meet your needs. This will help you decide whether you wish to take out additional coverage for you, your spouse and/or child(ren).
8.) At the end of the enrollment, you will have the option to print/email your confirmation statement for your record.
